Dr. Robert S. Liichow of Discernment Ministries International has an extensive collection of source materials about various heretical movements which he shares in his blog.
In “Five-Fold Fallacies” (Volume 10, Issue 10, October 2005), he traces the history of the “five-fold ministries” movement and its link to the “restoration,” “Latter Day Rain” and “manifest sons of God” movements (you’ll have to scroll down past other articles):

Sandy,
Thanks for the link. I’m familiar with Liichow’s work (Discernment Ministries is on my blog roll).
I also find a doctrinal link between those in the “five-fold” to New Age theology.
